Leaving hospital is an important milestone, but it is not always the end of someone's recovery.

Many people are clinically ready for discharge but continue to benefit from structured support while they rebuild confidence, develop independence and adjust to living back in the community.

At Oasis Care, we work with NHS teams, local authorities, commissioners and Community Mental Health Teams (CMHTs) to consider hospital step-down referrals for adults who require ongoing support following discharge.

Every referral is assessed individually to ensure our service is the right environment for both the person's immediate needs and their longer-term recovery.

When Might a Hospital Step-down Placement Be Appropriate?

Hospital step-down placements may be suitable for individuals who are clinically ready to leave an inpatient setting but would benefit from continued structured support before moving into fully independent living.

Our Referrals Team works alongside discharge coordinators, care coordinators, Community Mental Health Teams and other professionals to understand each person's circumstances before a placement is agreed.

Where we do not believe Oasis is the right service, we will explain this openly. Our priority is always finding the right placement, not simply the next available vacancy.
