Are there mobility or accessibility requirements?

How important is maintaining access to education, employment, family or existing professionals?

Are there established routines or community connections that should be maintained?

Does the proposed environment support the person's identified outcomes and long-term goals?

Medical & Clinical Support

Oasis can support people with a wide range of ongoing physical and mental health needs alongside their existing healthcare professionals.

Support may include:

Medication prompting or administration (where appropriate)

PRN medication support, subject to assessment and prescribing arrangements

Support to attend GP, hospital and specialist appointments

Liaison with Community Mental Health Teams (CMHT), Learning Disability Teams and other healthcare professionals

Monitoring of known physical and mental health conditions

Support with nutrition and hydration

Support to maintain healthcare routines and treatment plans

Monitoring for changes in presentation and escalation where appropriate

Does the person require medication prompting or administration?
Are there any PRN protocols or time-critical medication requirements?

We understand that emergency and urgent referrals may initially contain limited information. Where appropriate, we are happy to discuss referrals at an early stage and identify any additional information that may support the assessment process.
