We provide a secure and confidential way to share information, helping us gain an understanding of the individual's needs, aspirations, and support requirements.

Once we receive your referral, a member of our team will be in touch within 24 hours to discuss the individual's circumstances, answer any questions, and arrange an assessment at a convenient time.

Assess

Our experienced team carries out a comprehensive, person-centred assessment to understand the individual's strengths, preferences, goals, and support needs. We work collaboratively with the individual and those involved in their care to ensure we gain a complete picture of how best to support them.

Plan

Following the assessment, we develop tailored recommendations outlining how Oasis Care can support the individual to achieve greater independence, wellbeing, and quality of life. Recommendations are typically provided within 48 hours.

Welcome & review

Once a placement is agreed, we work closely with the individual, their support network, and relevant professionals to ensure a smooth transition. Regular reviews help us adapt support as needs change, ensuring the individual continues to thrive and progress towards their goals.
